WHO NEEDS A DOT MEDICAL CERTIFICATE?
- Any person operating a commercial vehicle that transports passengers or property for INTERSTATE (between states) commerce.
- Any person operating an INTRASTATE (within one state) vehicle that weighs over 10,000 pounds, carries 8 or more passengers for a fee, carries 15 or more passengers, or transports hazardous materials requiring vehicle signage.

DOT Exam Preparation Quick Guide
To insure you can receive your medical certificate immediately:
- Be ready to complete a health history.
- Be able to provide a small urine sample (this is not a drug test).
- Bring your glasses and/or hearing aids if you need them.
- Bring a list of your prescriptions and any OTC medicines used regularly.
- If you know you require an FMCSA waiver or assessment that needs your primary physician’s signature, obtain that before scheduling your physical and bring it with you.
- Heart disease and seizure episodes may require clearance from a cardiologist or neurologist. This can be discussed during your physical.

What Is a DOT Physical Exam?
The FMCSA requires a basic physical be performed by a certified medical examiner to help insure both driver health and public safety on our roads. The exam consists of a health history, brief physical, urinalysis, vision, and hearing checks, and can usually be completed in about 30 minutes. There are minimum health thresholds that must be met in order to qualify. Once passed, a driver is registered with the FMCSA database.
